Ematrix is only for early scarring and you need it monthly, this is the only device I say do ahead of time before manual methods. Older scars she can subcise, but that's 3 months apart so you can heal. Vbeam is for discoloration.

Hello, so I went to consult for some scar revision but u fortunately I hada breakout a couple days before so I was pretty much told to focus on the actives first. I decided to take accutane and will be starting in a month. The derm said that she believes the accutane will actually decrease the level of superficial scarring I have which is nice and that we can treat whats left over after the course is done, which I guess Im fine with. My only question is can I dermastamp within the month leading up to accutane? I have a feeling that if I do some stamping and then hop on accutane and increase the turnover of my skin Ill see some solid results. Does this sound like a feasible idea or should I just go on the accutane and wait it out?
Yes of course you can dermastamp a month before Accutane. In fact you can do a peel if you want at a med spa as well. Accutane changes the skin so much it's hard to say. Some people get more scars as the body purges that infection / sebum. Others have dryness and must heal 6 months after tane to see the final results which are better. It depends on the dosage, length of treatment, etc.

Great I will do that. Do you know if there are any indicators as to how bad the purge will be? I have heard things like it depends on how your skin reacts to the tretinoin and how bad your acne was before you started (ie relatively clear skin would result in no or minimal purge period.) Also one of the derms told me that at 135lbs I should start on 60mg while the other said 20mg. That's a really big difference and I'm not sure who to go with. Any insight?

I did treatments with Dr. Khrom in Brooklyn. She does sub with sculptra/prp.
@ViceroyLow dose should be fine 20mg, you can always try that once you get the medication, ... higher medication get's you results quicker and get's you done with tane quicker. But yes you get more side effects. It depends what gives you the effects your Dr and and you are looking fot 135lbs doesn't need a huge dose. Please be aware if you low dose at 20mg there is a purging period where things get worse - this is normal. Also as you build up on the medication in your body that is when you see the results. It all comes down to what you respond to and how quickly you want to be done.

I've actually seen 3 of 4 docs mentioned in this thread;I have rolling, boxcar, and icepick scars:
1. Dr Green - my first real derm in NYC. She treated my acne (accutane) and recommended Ematrix, Vbeam, and dermal fillers for my scars. I feel ripped off honestly since I did maybe $10k+ of treatments with her (multiple sessions of ematrix, vbeam, and filler) and had maybe 20% improvement, if any, of my scars. She also never checked in on how far I'm improving, so it felt like I'll just hafta come in forever and get treatments each month with her. WOULD NOT RECOMMEND.
2. Dr Geronemus - I went to him after Dr Green. He recommended Infini Genius for my scars. I'm extremely disappointed after seeing him for 1 treatment, since 1) his assistants didn't clean my skin before he started the treatment (it's summer in NYC - I had sunblock and sweat all over my face from walking outside), so it resulted in breakouts around the needle points, and 2) Dr Geronemus missed several scars on my face when he did the treatment, and spent very little time discussing my treatment plan with me (2 minutes of consult). After more research, it became very clear to me that his practice (nurses and him) are endorsing Infini Genius because they're in Infini's pockets; I believe my scarring would not magically improve just with microneedling RF, and would rather need a multi pronged approach (which the dr never mentioned).I came to him only because Dr Lim vouched for him.WOULD NOT RECOMMEND.
3. Dr Tzu- I went in for a consult only, but I think I'll move forward with her recommendation. When I went in for the consult, she spent about 20 minutes discussing the type of scars I have with me and her recommended course of treatment, and explained subcision to me. I can't vouch for her results yet, but I can update once I've gotten treatment.

She suggested I do a combo of subcision & microneedling.
I've done 1 subcision (with filler) and 1 microneedling session with her.
- Subcision cost: $1750USD
- Microneedling: $1000USD
I feel like there are subtle improvements to my skin, but nothing too noticeable yet. I'm hoping to do a few more microneedling sessions, and reevaluate if I should continue treatment
