I have a scar on the center of my nose from having a sebaceous adenoma lesion removed with hyrefractor 3 months ago. Can someone please tell me what type of scar this is (ice pick or box car), and what the best treatment options would be? I'm thinking subcision or tca CROSS, but I'm new here and I don't know a lot about treatments. Physician recommendations would be great as well - I live in the US (southeast). Thanks.
It looks like a small boxcar scar to me. Scars on the nose are tough to treat and you want to approach it very carefully and make sure you don't make it worse.
Just shooting from the hip (I'm not a doc!) I wonder if a very small amount of filler might be interesting for just one scar like that. Or perhaps a few quick fractional laser treatments and then a little filler.
Bottom line is you want to get under the care of a really good, experienced person and make sure you're comfortable before moving forward. Consider all your options and think about making sure the side effects are minimized. You don't want to make it worse through good intentions.

I've been using tazarotene cream .1% for several months along with skin better alpharet (twice weekly), rosacea combo cream(metronidazole 0.75%/azelaic acid 15%/ivermectin 1%), and sunscreen (Isdin Eryfotona Actinica) since I first posted. The scar seems shallower with less discoloration; however, I would like to see more improvement such as less depth so that it blends better with surrounding skin.
local treatment options:
-skin pen Microneedling with or without prp - I was thinking 3-6 sessions,
-Pixel8/VirtueRF/Morpheus8 Radiofrequency Microneedling,
-tetra fractional laser resurfacing
Treatments I feel may help, but are not offered locally, so I would need to travel include:
-tca or phenol cross technique if scar is deep enough
-fractional erbium or CO2 laser treatment
-subsicion (if scar is deep enough)
-punch excision (if scar is large enough)
Oh wow it's really improved. I would say leave it alone. The redness will subside. As I stated before, if you leave it alone there is no way you can mistakenly make it worse.
If you really wanted to do something, what about something non-ablative like a non-ablative laser? Just a little something to help with collagen and that has a low risk of sides.

I think most of the improvement for me was from tazaratene applied every night - at first I used the generic 0.1%, but switched to the weaker brand name Arazlo when I found out I could get it cheaper. I also use sunscreen (Isdin Eryfotana Actinica) and Cetaphil gentle skin cleanser twice a day. That is all I did to get the results in the pictures.
I recently completed 3 Skinpen microneedling sessions, stopped Arazlo, and started Accutane (isotretinoin) for an unrelated condition - I feel like I've seen more improvement in the scar. I'm not sure if it is the Accutane or microneedling that caused the improvement.
