Hello,

Which treatment would you think is the best for my acne scars?

I had a lot of treatments in the past: first full ablative co2 and erbium yag laser 9 years ago, than subcisions 7-8, ha filler 4-5 and sculptra 2x, rf microneedling 2x, deep microneedling 3-4, fat grafting.

I dont know what i can do now. I want to do something invasiv. Maybe 1 more full ablative co2 laser and erbium yag (good results 9 years ago) or dermabrasion with prp for some new scarring? Dermabrasion have greater results than full ablative co2? And more subcisions with sculptra for my fat loss?

Hey. Been busy and just saw this. From the before and afters in my research (not real life! just in studies!) I am most impressed with ablative fractional laser + PRP, and then giving it many sessions - say 6 or even more. I don't think one session of anything will probably give you the results you're looking for.

Dermabrasion can give good results but it scares me because it's prone to human error. Lasers are more controlled. But in the best hands, dermabrasion can produce great results.

Also, just as an aside, remember that a lot of scar repair is acceptance. You can't really get rid of scarring once you have it. You can only look for improvement. Imperfections are human and so while you are working on doing more for your scarring, you might want to also work on acceptance. Embrace your imperfections if you can.

Without question dermabrasion helped me when I was young the only thing that worked, but created textural problems (although I'd take them over the alternative) but also as you say is very, very skill based. It's so skill based that it could change pigment if a bad doctor does it. Also, no doctors do aggressive dermabrasion anymore.

Fractional didn't work well for me, I had a test spot. I wonder if microneedling is better. For the OP it's mostly rolling scars and textural stuff. I have to assume fractional/PRP or other ablative laser to smooth it out would be the best place to look but it should be someone skilled.
